Geriatric Case Management

The Office of Senior Citizens Services contracts with outside agencies to provide case management services.

The geriatric case managers are trained social workers who are dedicated to assist frail elders and their families in constructing appropriate living arrangements, managing day-to-day needs and long-term supportive services. 

Case managers are knowledgeable of available services/resources in Jefferson County.  They provide on-going and short-term case management services as indicated in the assessment.
