Change of Name for an Adult

  • You must be 19 or older to petition for a change of name.
  • You must fill out a Petition and Declaration for Change of Name in black ink only.
  • Each area marked 'Petitioner" on the form refers to your present legal name. Be sure to sign your present legal name as 'Petitioner' and have that same signature notarized. Use full names. Do not use initials.
  • Be sure to give your physical address. No post office boxes.
  • Bring the completed, signed, notarized forms to the office of the Judge of Probate along with a filing fee of $11.00 in cash. No checks or credit cards will be accepted.
  • If you wish to mail the forms in, remit a money order in the amount of $11.00 made payable to the Judge of Probate. Again, no personal checks will be accepted.

Change of Name for a Minor

  • On the minor child's behalf, you must fill out a Petition for Change of Minor's Name in black ink only.
  • We must have a certified copy of the child's birth certificate supporting the Alabama State Identification Number. This must remain a permanent part of our file. If both parents' names appear on the birth certificate, then both parents must petition to have the child's name changed. If one parent is not available to sign the petition, or if the fathers name is not listed on the birth certificate, then Item # 2 on petition must be completed in full.If the minor child is fourteen (14) years of age or older, then he/she must submit a consent to the change of name. This may be an informal handwritten statement requesting his/her name be changed, but his/her signature must be notarized.
  • Signature(s) of the petitioner(s) must also be notarized. Use full names. No initials.
  • Be sure to give your physical address. No post office boxes.
    Bring the completed, signed, notarized forms to the office of the Judge of Probate at the address above with the filing fee of $11.00 in cash. No checks or credit cards will be accepted. (Please be aware that there will be additional charges if the matter is set for hearing.)
