All services at Cooper Green Hospital, except for the Community Care Plan, are available on a sliding fee schedule to persons who have a HealthFirst card. Fees are based on family size and income. The requirements for a HealthFirst card include:
- Documentation of residence in Jefferson County
- Proof of income
- Social Security Card
- Photo Identification
