Cooper Green Hospital
The Community Care Plan is a health insurance program for uninsured persons living in Jefferson County. All services are provided through one of the five neighborhood-based clinics to persons who are members of the Community Care Plan. Annual membership premiums are based on family size and income. Family and individual plans are available.
Family Membership Example: A family of four (4) with an annual income of $15,000 pays an annual membership fee of $342.
Individual Membership Example: A person with an annual income of $10,000 pays an annual membership fee of $171.
A small copayment is required for each visit and each prescription that is filled through the Community Care Plan. A unique health promotion program, HealthPoints, allows members to earn discounts on their premiums through an individually designed program.
